Which span or region are these located in, in reference to the physical world?

Hell
Beginning with the physical world which we are using at this time, there is below it subterranean astral regions which are places of negative gravity, meaning that these places exist in a realm where the matter is subtle and yet it is heavier than the matter which we perceive. The hellish astral regions are located in these domains. Physically if something is denser, more compact, then you are sure to see it but in this case these realities are denser and more compact and still cannot be seen physically because they are made of a frequency of energy which is not perceptive to our present range of vision.
Heaven
Heaven is in the astral world but it is made of a lighter material than what we are experiencing now. It is more like a place which is made of sunlight and moon light, while the hellish astral world is like a place which is made of X-rays. Both sunlight and X-rays are a sort of frequency energy and still we cannot see X-rays.
Dream
Dream, when it is an astral occurrence happens either in the hellish or heavenly dimensions. When dreams are merely the imagination of the person, then it is made of the person’s mental energy which is procured by the mind according to the frequency in which the mind operates at the said time.
To define what level the dream occurs on, we must first determine if it is an astral experience or a mental construction in the mind of the person, or even if it is a mixture of the two.
For instance a boy sits at a table and eats a cookie. Others observe this and see the boy doing this. Another boy sits at another table where there is no food but this boy is mentally occupied imagining himself eating a cookie. And then there is yet another body who sits at another table who while eating an actual cookie, also imagines that he is eating ice-cream with the cookie, even though no ice-cream is present.
Imagination
Imagination is the working of the mind to create an image within the mind itself irrespective of reality.

According to Hindu lore, below Muladhara chakra (earth chakra), then subterranean, there are 7 underworlds or hells: Atala, Vitala, Sutala, Talatala, Rasatala, Mahatala and Patala Lokas.
Muladhara, is in Bhu Loka or earth plane.
Ascending with the chakras, there are Bhuvah, Svah, Maha, Jana, Tapa and Satya Lokas.
Satya Loka or Brahma Loka is supposed to be a pure spiritual realm, above the Trimurti (Brahma, Vishnu and Shiva).
In Satya Loka resides Swami Parambrahma, which is mentioned in Swami Shriyukteshwar's book "Kaivalya Darshana".

Imagination does not give anyone the right to go wherever they wish or to do whatever they desire unless that person is in a realm of consciousness which permits that.
Imagination is relative and circumspect in reference to the allowances of fate for the particular dimension the person is in. For instance, if the president of India imagines a nuclear war with Pakistan, there is a likelihood that this might happen because the president has certain executive powers through which he can order the military to release a bomb. But if I decide to do this or if you decide to this, it will remain as our local imagination only and nothing will happen. Reality will just ignore our imaginative powers.
First we have a responsive environment and then our imagination will work to do wonders, otherwise it remains only something which is an isolated mental or emotional action within our individual minds.
In the astral world hereafter, you or I may find ourselves in an astral dimension in which the environment responds to our imaginations, such that if I want to live in a dhobi hut, it will be there and if you want to live in a Taj Mahal, it will be there. However this is not happening because of our power. It happens because that environment was already there with responsive attitude to our energy.
You may go out today and purchase an expensive automobile, which has some system of wiring which connects to your brain and which responds to your thoughts but there you cannot claim to have manufactured the car. Your claim would be that someone else did that and it so happened that it was designed to adapt to your wishes.
What are the limits of human imagination?
It is limited by the response index of the already created environment the person happens to be in.

What is imagined in the local area of the individual mind may or may not affect what is beyond the individual’s mental jurisdiction. As in the example above, the president’s mind might extend its powers into the environment so as to affect the whole of India, while your mind or my mind might not have the extension influence to affect anyone besides ourselves, our friends and family.
Mental construction does have boundaries but that is determined by the response index in the environment as I explained above. In any case a limited individual, you or me, does not, at any stage, have unlimited power, which means that to some extent our influence will always be limited? The degree of limitation is not determined by our mental power but by the response index of the environment. The same cannot be said for the Supreme Being because that Being is in a totally different category being able to produce the environments which sponsor or deny our desires.
In this world we see that a very wealthy individual can own a manufacturing concern which produces phones for the population of the whole country, while a poor man cannot even manufacture one screw on the said phone. In this world we do have some examples of this.

The first method of doing this is to cease all visualization and imagination during meditation sessions. In other words make it your priority to honor the second sutra of Patanjali which is
Yogah cittavritti nirodhah
Which I translated as:
The skill of yoga is demonstrated by the conscious non-operation of the vibrational modes of the mento-emotional energy.
As soon as this ceases, then there will be internal honesty. One will be able to know what is dream imagination and what is astral reality.
However mastery of Patanjali’s instruction may take more than a life time, so we may begin by taking the kindergarten steps which we learned in this life in infancy. At time when it was time to cease crawling and creeping, we stood up, held on to something and then stepped out only to find that it was a miscalculation. The place where we felt it was safe to put the foot proved to be unsafe. By that we learned that we had to properly gage distance or fall and be hurt and embarrassed besides. We learned how to calibrate our imagination so that it matched reality.
Can we go back in memory and study what happened then, as to how it came to be that we are actually walking now in a balanced way automatically. It seems that our imaginative faculty learned how to make itself confirm to reality.
Right now, how do you know if you are imagination something or if it is a physical reality? For instance if you are hungry how do you determine if you are eating an actual meal or if you are sitting imagining yourself eating such a meal. How do you know the difference?
If you have a method then this same method can be applied to differentiate between dream imagination and astral occurrence.
Obviously it has something to do with self honesty. Most of us have a self-dishonesty factor which is so high that we can hardly tell when we are imaging and when we are actually perceiving some astral or psychic reality. We neither have confidence in our astral perception or in anyone else’s, because of this innate dishonesty. But you can over time train yourself and rise above it.
A good beginning point is to make a commitment to never imagine or visualize during meditation. Of course, that is easier said than done, but you should start somewhere. The mind you have may not obey your command and it will keep on imagining and visualizing during meditation but you should have an attitude of non-indulgence and then in time perhaps you may reach the stage where the mind ceases this cittavritti activity.

I don’t know where you are getting this information from about no above and no below. According to modern astronomy, the North Pole is up and the South Pole is down. There is an upper and lower dimensional alignment to the universes according to the rotating axis of the galaxies and each of the galaxies have an up and down.
You might be confusing the gravitation pull of the earth with this axis system.
However in respect to negative gravity you have to get to that when your meditation practice reaches a higher stage, because that cannot be proven by science or by objective physical means. Just as we are being kept on the earth by the earth gravitational field, there are forces which can repel us from the earth just as well but that is psychic stuff which is outside of the usual way of scientific investigation.
If you need scientific proof every step of the way, then your best bet is to deny psychic stuff and just stick to science.

In terms of dimensions there are higher and lower, depending on the kind of energy used in those supernatural places. For this subjective evidence is required and that is different to the methods used in science.
